Are you working with an individual who is looking for safe, stable, and affordable shared or private housing? Community Care Homes provides structured shared housing for adults who are able to live independently and are seeking a supportive community and a place to call home.

Who Can Be Referred?

We welcome referrals for adults who may be seeking housing due to:

  • Transitioning from a hospital, rehabilitation facility, or other temporary setting
  • Loss of housing or unstable housing
  • Transitioning to independent living
  • Fixed-income housing needs
  • Social Security, SSI, SSDI, or VA income
  • Support from a family member, case manager, social worker, or representative payee

What Community Care Homes Provides

Participants may have access to:

  • Furnished shared and private rooms
  • Utilities, Wi-Fi, and household essentials
  • Light housekeeping of common areas
  • A safe, structured, drug-free and alcohol-free living environment
  • 24-hour surveillance
  • Community resources
  • Optional services and support based on individual needs
